Send completed form and $2.00 per transcript (cash or money order only) to:

Telephone orders are not accepted.
Students currently registered at Cypress Bay will have to order their transcripts during school hours, the request forms are located in Guidance in the Registrar's Corner.
Hard copy transcripts are $2 each.
Electronic transmission (Florida State Public Universities only) are free of charge.
